Desk Chart

A desk chart is a way to monitor student progress on a behavior goal. It can be used as a method of feedback to allow a child to gauge his success in modifying problem behavior.

Using tally marks, a student will receive a mark when he is following target behaviors. He will need to have three tally marks after each activity to receive a sticker for that class period.

Below is an example Desk Chart for CW from the Sample Behavior Management Plan list of target replacement behaviors.

If he receives three stickers at the end of the day, he will be able to select a favorite activity from the reward survey to complete during activity time.
